Vaccination Appointment Instructions
We are excited to host three more Vaccination Clinics at HSHMC thanks to Sharp Healthcare! The next clinic is October 4, from 10:00 to 2:00PM. Future dates include 10/25 and 11/15. See below for instructions to make your appointment.
If your child is already fully vaccinated, please remember to provide the school with a copy of the vaccination card for our records. Feel free to take a picture of the card and email it to gortega@hshmc.org, mail it in, or drop it by. Any way that is easiest for you to get it to us as soon as possible.
Step 1: Go to https://myturn.ca.gov/
Step 2: Click “Make an Appointment”
Step 3: Fill out the fields on the “Let’s Get Started” page
Step 4: Enter the school’s zip code: 92105
Step 5: Click “Schedule Appointment on the “Select Location” page. You will see HSHMC as an option.
Step 6: Select the time that works best for you on the day of the Dose 1 vaccination event, then select the time that works best for you on the day of the Dose 2 vaccination event. Then, click “Continue.”
Step 7: Fill out your personal and contact information, and then click “Complete Appointment.”
